Re: Request for reasonable accommodation

Dear Recipient name,

I am writing to request a reasonable accommodation under the Americans with Disabilities Act. I have a medical condition that affects my ability to describe the job function affected.

To perform the essential functions of my job as Your job title, I am requesting the following accommodation: accommodation requested, e.g. modified schedule, ergonomic equipment, remote work.

I believe this accommodation will allow me to continue performing my duties effectively. I am happy to provide supporting documentation from my healthcare provider and to discuss alternatives.

Please let me know when we can meet to discuss this request.

Sincerely,
